The first idea that came to my mind after watching Pepsi's unfortunate ad was, who on earth advised them? You would think that they would show the ad to different teams and departments. How could they be blind to what we all saw so crystal clear? 

Once I found out that the ad had been created by Pepsi's in-house agency I wondered: Would this have happened if they had worked with and external agency, maybe more aware of the bigger picture?
